More than 100 workers are killed and thousands more are seriously injured in forklift accidents every year — and the victims are split between the operators themselves and the pedestrians working around them. That’s a sobering number for a machine most crews treat as routine. The truth is that a forklift is nothing like a car: it’s twice as heavy, it steers from the rear, it pivots on a narrow wheelbase, and it’s deliberately built to carry loads that shift its balance point with every lift. What OSHA says about forklifts
OSHA classifies forklifts as powered industrial trucks, covered by 29 CFR 1910.178. On construction sites, 29 CFR 1926.602 governs material handling equipment, and forklift operator training follows the requirements of 1910.178(l): formal instruction, practical hands-on training, and an evaluation before anyone operates the truck alone — plus a re-evaluation at least every three years.

The stability triangle: why forklifts tip
Draw an imaginary line connecting the two front wheels and the pivot point of the rear axle. That’s the stability triangle — a map of the forklift’s suspension. Keep the machine’s center of gravity inside the triangle and it stays upright. Let it drift outside — through speed, a sharp turn, a raised load, or an oversized load center — and the truck goes over.
The machine works like a seesaw balanced on the front axle:

Height matters too. Raising the forks — even empty — raises the truck’s center of gravity, which is why you travel with forks 4 to 6 inches off the ground, mast tilted back, and never turn with them raised higher than that.
Know your load capacity and load center
Every forklift has a rated load capacity tied to a load center — the distance from the fork backrest to the load’s center of gravity. The standard is 24 inches, half the depth of a standard pallet. Here’s the trap: a truck rated for 4,000 pounds at a 24-inch load center can tip forward with a 4,000-pound load at a 36-inch load center, because the longer load shifts the combined center of gravity toward the front wheels. When a load is non-standard, rearrange the pallet or reposition the forks to bring the load center in — and restack anything badly stacked or sitting on a damaged pallet before you lift it.
When engaging a load: square up to the pallet, slide the forks in until the load touches the backrest (forks should be at least two-thirds the load’s length), center it side to side, lift only as high as needed, and use just enough back tilt to stabilize it.
Safe driving habits that prevent accidents
Struck-by and caught-in/between incidents — two of construction’s Focus Four killers — are exactly how forklift accidents hurt pedestrians. The habits below are the countermeasures:
- Buckle up every time, and don’t drive if medication, fatigue, anger, or anything else is dulling your focus. Never operate under the influence of drugs or alcohol.
- Keep it to walking speed — about 5 mph. It keeps the truck stable and gives you time to react.
- Come to a complete stop before changing direction, then turn slowly and smoothly. Remember the center of gravity swings to the outside of every turn.
- If the load blocks your view, drive in reverse with the load trailing so you can see where you’re going.
- Sound the horn at intersections, doorways, and blind corners, make eye contact with pedestrians, and give them the right-of-way. Never let anyone walk or stand under raised forks, ride on the forks, or ride on the load.
- Stay at least three truck lengths behind other forklifts, and don’t pass at intersections.
- Keep the mast and forks at least 10 feet from electrical wires and equipment — they conduct electricity.
- On ramps and grades, keep a loaded truck’s forks pointed uphill (drive up forward, back down) and an unloaded truck’s forks pointed downhill. Never turn on or drive across an incline.
- On docks, verify trailers are braked and chocked, check that floors can hold your truck plus its load, and use a secured, properly rated dock plate.
Remember: if your forklift starts to tip, do not jump. Studies show jumping is what gets operators killed. Brace your feet, grip the steering wheel, pull yourself tight against it, lean away from the tip, and hang on.
When you park: forks flat on the ground, parking brake set, motor off, keys with you — and wheels chocked on any incline.
The pre-shift inspection
OSHA expects powered industrial trucks to be examined before being placed in service, and a faulty truck comes out of service — no exceptions. A solid walk-around covers:
- Obvious damage, missing parts, and fuel or oil leaks
- Fluid levels — fuel, engine oil, transmission, coolant (or battery electrolyte on electric trucks, wearing eye protection, a face shield, and rubber gloves, since it’s corrosive)
- Tires — damage and embedded objects
- Steering, service brakes, and parking brake
- Mast chains, bearings, nuts, and cotter pins; hydraulic oil level and leaks
- Forks — cracks, excessive wear, alignment
- Lights, turn signals, flashers, horn, backup alarm, warning beacon, and seatbelt
Refueling and recharging carry their own fire risk — gasoline, diesel, and propane are flammable, and charging batteries generate hydrogen gas. Work only in well-ventilated areas with no ignition sources, shut the engine off first, keep metal tools (and jewelry) away from battery terminals, and leave the battery compartment open while charging.
